Passport and Travel Document

A. RENEWAL

General Requirements:

  1. Personal appearance of the applicant;
  2. Duly accomplished passport application form;
  3. Original passport and photocopy of data page;
  4. Original Permesso di Soggiorno (Permit to Stay) or Carta d'Identità (Identity Card);
  5. Payment of the application fee of €54,00.

Additional Requirements:

If applicant is a minor:

  1. If applicant is minor, personal appearance of either parent;
  2. Original and photocopy of either or both parents’ passport data page and amendment page, if applicable.

If there is no Permesso di Soggiorno and/or Carta d'Identità:

  1. Original PSA-issued Birth Certificate;
  2. Original PSA-issued Marriage Certificate, if married.

If applicant’s Machine Readable Passport (MRP) does not indicate the municipality or city of the passport holder’s place of birth (ex. TARLAC, instead of TARLAC, TARLAC or METRO MANILA instead of SAN JUAN, METRO MANILA):

1. Original PSA-issued Birth Certificate or original PSA-issued Marriage Certificate which indicates the place of birth of the applicant.

If applicant is a married woman who would like to use her married name:

1. Original PSA-issued Marriage Certificate; or
2. Original Marriage Contract, if solemnized in PCG Milan; or
3. Original Report of Marriage, if marriage was solemnized in Italy and reported to PCG Milan.

If applicant is a married woman who would like to revert to her maiden name:

  1. If already widowed: Original PSA-issued Death Certificate of the husband.
  2. If marriage is annulled: Original of
    a) PSA-issued annotated Marriage Contract, and
    b) Court Order effecting the annulment.
  3. If divorced:
    a) Certified True Copy (CTC) of the Divorce Decree duly authenticated by the Philippine Embassy or Consulate where the divorce was obtained or by the concerned foreign diplomatic or consular mission in the Philippines;
    b) Original and CTC of Philippine Court recognition of the foreign divorce decree;
    c) Original PSA-issued Marriage Contract with the annotation of the Divorce Decree.

B. FIRST-TIME APPLICANT

Requirements:

  1. Personal appearance of the newborn / first –time applicant;
  2. Personal appearance of either parent if new born is a legitimate child. If illegitimate but recognized by the father, personal appearance of both parents;
  3. Duly accomplished application form;
  4. Report of Birth;
  5. Payment of the application fee of €54.00.

C. REPLACEMENT OF LOST PASSPORT

Requirements:

  1. Personal appearance of the applicant;
  2. Duly accomplished application form;
  3. Affidavit of Loss;
  4. Original Police Report/Denuncia;
  5. Original PSA-issued Birth Certificate;
  6. Original PSA-issued Marriage Certificate, if married;
  7. Photocopy of lost passport, if available;
  8. Original Permesso di Soggiorno and/or Carta d'Identita, if available;
  9. If applicant is minor, personal appearance of either parent;
  10. Payment of the application fee of €135.00 (for lost ePassport) or €81.00 (for lost Machine Ready Readable Passport or Machine Readable Passport).

Note: A 15-day clearance period is mandatory for applications for replacement of lost passports.

D. REPLACEMENT OF MUTILATED OR DAMAGED PASSPORTS:

Requirements:

  1. Affidavit of Explanation / Sinumpaang Salaysay;
  2. Original mutilated or damaged passport and amendment page, if applicable;
  3. Original PSA-issued Birth Certificate;
  4. Original PSA-issued Marriage Certificate, if married;
  5. Original Permesso di Soggiorno and/or Carta d'Identita, if available;
  6. Payment of the application fee of €54.

Note: In any of the aforementioned passport transactions, the evaluating consular officer may still ask the applicant to produce and present a DFA-authenticated PSA-issued civil registry document if, after careful evaluation of the application, there is reasonable basis to require such document in order to protect the integrity of the Philippine passport.

IMPORTANT REMINDERS:

  1. Applicants should wear collared shirts when they apply for their passports.
  2. The release date of the passport will be indicated in the applicant's official receipt.
  3. Passports which are unclaimed after six (6) months from the date of issuance shall be cancelled per Department of Foreign Affairs Order No. 37-03.
  4. Passports shall only be released to the applicant or his/her representative (with proper identification and written authorization signed by the applicant). Passports of applicants below 18 years old can be claimed by the parent/guardian who submitted and signed the application. To claim the passport, the previous passport, official receipt, and authorization must be presented.
